Currently, I am an in-network preferred provider with the following insurance companies:
Aetna, Asuris, Group Health/Kaiser Permanente, United Behavioral Health, United Healthcare - Private, Providence Behavioral Health and HMA.
If you do not see your insurance provider listed above, I will be happy to work with you as an out-of-network provider and file the claims for you. I currently am able to see people with all insurance plans - your co-payment will be based upon what plan you have.
When you call the office to make an appointment we will discuss insurance issues and what you can expect.
I ask all clients to be aware of the following information:
- Knowing your insurance policy, what your mental health benefits are, if you have a deductible amount and if that has been met, and your co-payment or co-insurance.
- You are expected to pay your co-pay/co-insurance or your fee at the time of service.
Other billing information will be reviewed during our first phone contact or during our first session. You will be asked to sign a payment contract outlining your responsibilities regarding payment.
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you may be required to pay the full cost of the session.
